Location: Remote Annual Salary: $54,528 Job Overview: We seek a dedicated and skilled Virtual Patient Care Manager to join our dynamic team. Key Responsibilities:
  1. Patient Coordination:
    • Serve as the primary point of contact for patients, facilitating communication between patients and healthcare providers.
    • Develop and maintain comprehensive care plans tailored to individual patient needs, ensuring all aspects of care are addressed.
    • Schedule and manage virtual appointments, follow-up calls, and other patient interactions to support ongoing care.
  2. Care Management:
    • Monitor patient progress and provide timely interventions to address any issues or concerns.
    • Regular patient needs assessments should be conducted, and care plans should be adjusted to optimize outcomes.
    • Ensure adherence to treatment protocols and guidelines by reviewing patient records and collaborating with healthcare providers.
  3. Patient Advocacy:

  6. Technical Proficiency:
Qualifications:
  • Education: Bachelor's degree in Nursing, Healthcare Administration, or a related field is required. A Master's degree is preferred.
  • Experience: Minimum of 3 years of experience in patient care management, preferably in a virtual or remote setting.
  • Certification: Relevant certifications, such as Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ), are highly desirable.
  • Skills:
    • Exceptional communication and interpersonal skills with the ability to build rapport and trust with patients.
