Key Responsibilities:- Patient Interaction and Support:
- Engage with patients through virtual platforms (video, phone, chat) to assess their needs, answer questions, and provide guidance on health management.
- Conduct initial and follow-up consultations to address patient concerns and monitor progress.
- Offer emotional support and encouragement to patients navigating their healthcare journeys.
- Care Coordination:
- Collaborate with healthcare providers to develop and implement personalized care plans based on patient assessments and medical history.
- Facilitate communication between patients and their healthcare teams to ensure continuity of care.
- Schedule and coordinate virtual appointments and follow-up visits, ensuring timely and efficient patient care.
- Health Education:
- Provide patients with accurate and relevant information about their conditions, treatment options, and preventive measures.
- Educate patients on managing chronic conditions, medication adherence, and lifestyle modifications.
- Develop and share educational materials and resources to enhance patient understanding and self-care.

- Experience: Minimum of 2 years of experience in a healthcare or patient care role, with expertise in virtual or remote settings preferred.
