Key Responsibilities

One-on-One Coaching

• Conduct virtual sessions to coach clients on managing their diabetes effectively
• Foster trust and accountability through regular goal-setting and motivational check-ins
• Empower clients to develop habits that support long-term wellness

Patient Education

• Deliver tailored education on managing Type 1, Type 2, and pre-diabetes
• Break down complex medical concepts into accessible, actionable information
• Educate clients on the role of diet, physical activity, medication, and stress management in diabetes care

Progress Monitoring

• Track biometric data such as glucose levels, A1C readings, and blood pressure
• Analyze progress reports to refine individual care plans
• Provide real-time feedback to help clients stay motivated and informed

Team Collaboration

• Work collaboratively with registered dietitians, endocrinologists, nurses, and behavioral therapists
• Share insights in team huddles and case review meetings
• Recommend strategies for holistic care delivery based on patient insights

Documentation & Reporting

• Accurately document session notes, client updates, and key metrics
• Maintain compliance with privacy standards and digital health protocols
• Generate data summaries to support client reviews and clinical assessments

Qualifications & Skills

✅ Educational Background

• Bachelor’s degree in Nursing, Nutrition, Public Health, Behavioral Health, or related discipline (Required)
• Certified Diabetes Care and Education Specialist (CDCES), NBC-HWC, or equivalent health coach certification (Preferred)

✅ Professional Experience

• At least 2 years of experience in health coaching, diabetes management, or clinical patient care
• Experience with motivational interviewing and goal-oriented coaching methods

• Strong analytical and decision-making capabilities
• Comfort with telehealth tools and remote engagement platforms
• Ability to multitask, organize, and track outcomes effectively
